Accident Waiver and Release of Liability for Soccer Training with Charles Lawrence Please read the following information carefully before your child participates in the soccer training program provided by Charles Lawrence. Understanding the Risks Participating in any physical activity involves inherent risks. Soccer, despite being a controlled sport, can still lead to injuries. This waiver outlines the potential risks associated with participation in the program and aims to ensure you understand your responsibilities as a parent/guardian. Important Note: This soccer training program is conducted by Charles Lawrence. The coaching services are the sole responsibility of Charles Lawrence. Waiver of Liability By allowing your child to participate in the soccer training program with Charles Lawrence, you agree to the following: Assumption of Risk: You acknowledge and assume all risks associated with your child's participation in the program, including those arising from negligence or unforeseen circumstances related to the training activities. Medical Fitness: You certify that your child is physically fit and has no medical conditions that would preclude safe participation in the program. You are responsible for ensuring Charles Lawrence is made aware of any relevant medical conditions or limitations. Release of Liability: You waive, release, and discharge Charles Lawrence, his assistants, any designated training staff from any and all liability arising from your child's participation in the program, including injuries, property damage, or any other unforeseen events directly related to the training. You understand and agree that Charles Lawrence is solely responsible for the provision of coaching services and any liability arising therefrom. Indemnification: You agree to hold harmless and indemnify Charles Lawrence from any claims or lawsuits resulting from your child's participation in the program. Medical Treatment Consent: You grant permission for Charles Lawrence or his designated program staff to seek and approve medical treatment for your child if deemed necessary in the event of an injury or illness during the training sessions. Photo/Video Release: You understand that photos and videos may be taken during the program for promotional purposes related to Charles Lawrence's soccer training programs.
