Fairo Sport - Accident Waiver and Release of Liability for Leo Peralta Coaching

Please read the following information carefully before your child participates in the soccer program coached by Leo Peralta.

Understanding the Risks

Participating in any physical activity involves inherent risks. Soccer, despite being a controlled sport, can still lead to injuries. This waiver outlines the potential risks associated with participation in the program and aims to ensure you understand your responsibilities as a parent/guardian.

Important Note: This program is coached by Leo Peralta. Fairo Sport is providing the facilities or organizational support but is not responsible for the coaching services provided by Leo Peralta.

Waiver of Liability

By allowing your child to participate in the soccer program coached by Leo Peralta, you agree to the following:

Assumption of Risk: You acknowledge and assume all risks associated with your child's participation in the program, including those arising from negligence or unforeseen circumstances.
Medical Fitness: You certify that your child is physically fit and has no medical conditions that would preclude safe participation in the program.
Release of Liability: You waive, release, and discharge Leo Peralta, his employees, volunteers, and agents from any and all liability arising from your child's participation in the program, including injuries, property damage, or any other unforeseen events. You further acknowledge and agree that Fairo Sport, its directors, officers, employees, volunteers, and agents are not responsible for any liability arising from the coaching services provided by Leo Peralta.
Indemnification: You agree to hold harmless and indemnify Leo Peralta from any claims or lawsuits resulting from your child's participation in the program. You further agree to hold harmless and indemnify Fairo Sport from any claims or lawsuits arising from the coaching services provided by Leo Peralta.
Medical Treatment Consent: You grant permission for program staff (under the direction of Leo Peralta or Fairo Sport, if applicable in an emergency) to seek medical treatment for your child if deemed necessary in the event of an injury or illness.
Photo/Video Release: You understand that photos and videos may be taken during the program and agree to their use for promotional purposes related to Leo Peralta's soccer programs and potentially Fairo Sport's facilities or programs.
This revised waiver clearly indicates that it's for Leo Peralta's coaching and explicitly states that Fairo Sport is not responsible for his coaching. Make sure this is legally reviewed for your specific needs and jurisdiction.
